Delaware Hospice Achieves National Recognition through ‘Quality Connections’ Program

Delaware Hospice has achieved recognition by earning four rings in the 2024 Quality Connections program of the National Alliance for Care at Home (the Alliance).

Quality Connections is structured around four fundamental pillars, which are represented by four rings: Education, Application, Measurement, and Innovation. Participants engage in activities such as quarterly data reporting and benchmarking, educational courses, case studies, and engagement on emerging issues, all of which serve to promote high-quality care delivery and service excellence. Achievement in the program is measured by completing activities within each of the four foundational areas, culminating in the closure of up to four rings.

Delaware Hospice has earned the highest achievement possible in the Quality Connections 2024 program year by closing all four rings. The accomplishment underscores Delaware Hospice’s commitment to a continuous quality improvement program and educating your team on the newest and best practices in serious illness and end-of-life care.

Quality Connections began as a program of the National Hospice and Palliative Care Organization, which has joined together with the National Association for Home Care & Hospice to form the Alliance. Founded as a program focusing on hospice and palliative care providers, Quality Connections will expand in 2025 under the Alliance to provide quality improvement tools and support to the full care-at-home continuum.
